Ketorolac tromethamine is a potent non-steroidal anti-inflammatory drug (NSAID) belonging to the heteroaryl acetic acid derivative family, primarily indicated for the short-term management of moderate to severe acute pain. It exerts its analgesic, anti-inflammatory, and antipyretic effects by inhibiting the enzymes cyclooxygenase-1 and 2 (COX-1 and COX-2), which in turn prevents the synthesis of prostaglandins. Due to its exceptionally high analgesic potency—which is comparable to some opioids but without the associated respiratory depression or physical dependence—its use is strictly limited to a maximum of five days. This restriction is necessary to mitigate the high risk of serious adverse effects, particularly gastrointestinal ulceration, perforation, and renal impairment. It is frequently utilized in postoperative settings and is available in oral, ophthalmic, and injectable formulations to provide rapid systemic relief.
