Clopidogrel Bisulphate is an oral, second-generation thienopyridine antiplatelet agent utilized to reduce the risk of myocardial infarction, ischemic stroke, and vascular death in patients with atherosclerosis or acute coronary syndrome. As a prodrug, it requires hepatic bioactivation via the cytochrome P450 enzyme system (specifically CYP2C19) to form its active metabolite, which then irreversibly binds to the P2Y12 adenosine diphosphate (ADP) receptor on the platelet surface. This action prevents the activation of the glycoprotein IIb/IIIa receptor complex, effectively inhibiting platelet aggregation for the remainder of the platelet's lifespan. While highly effective in preventing stent thrombosis and secondary cardiovascular events, its efficacy can be influenced by genetic polymorphisms in the CYP2C19 enzyme, and patients must be monitored for potential bleeding complications.
