Prazosin hydrochloride is a selective alpha-1 adrenergic receptor antagonist primarily used in the management of hypertension and the symptomatic treatment of benign prostatic hyperplasia (BPH). By competitively inhibiting alpha-1 receptors on vascular smooth muscle, it induces peripheral vasodilation, which reduces total peripheral resistance and lowers blood pressure without typically causing significant reflex tachycardia. In the context of BPH, it relaxes the smooth muscle of the bladder neck and prostate, improving urinary flow. A unique clinical characteristic of prazosin is the "first-dose phenomenon," where the initial dose can cause a precipitous drop in blood pressure and syncope, necessitating a low starting dose usually administered at bedtime. Beyond its cardiovascular applications, it is also frequently prescribed off-label to reduce the frequency and intensity of trauma-related nightmares in patients with Post-Traumatic Stress Disorder (PTSD) due to its ability to modulate the central noradrenergic response.
