Levosalbutamol (also known as levalbuterol) is a potent short-acting beta-2 adrenergic receptor agonist used primarily as a bronchodilator for the management of asthma and chronic obstructive pulmonary disease (COPD). As the pure (R)-enantiomer of the racemic mixture salbutamol, it functions by relaxing the smooth muscles of the airways, thereby increasing airflow to the lungs during episodes of bronchospasm. The clinical advantage of using the isolated (R)-isomer lies in its higher affinity for beta-2 receptors compared to the (S)-isomer, which is traditionally considered inert but may be associated with pro-inflammatory effects or increased side effects.
