Calcium carbonate is a versatile chemical compound and medication widely utilized as a calcium supplement, antacid, and phosphate binder. As an antacid, it works through a direct chemical neutralization of gastric acid; upon ingestion, the carbonate salt reacts with hydrochloric acid (HCl) in the stomach to form calcium chloride, water, and carbon dioxide, thereby increasing gastric pH and providing rapid relief from symptoms of heartburn, acid indigestion, and dyspepsia. In its role as a mineral supplement, it is essential for the prevention and treatment of osteoporosis, osteomalacia, and rickets, as calcium is a critical structural component of the bone matrix and is necessary for neuromuscular function, blood coagulation, and enzymatic processes. It is also frequently prescribed in cases of chronic kidney disease (CKD) to manage hyperphosphatemia, where it binds to dietary phosphate in the gastrointestinal tract to form insoluble calcium phosphate complexes that are excreted in the feces. Absorption of calcium carbonate is pH-dependent and most efficient in an acidic environment, making it more effective when taken with food. Common adverse effects include constipation, flatulence and, in cases of excessive intake, the risk of hypercalcemia or milk-alkali syndrome. It is contraindicated in patients with hypercalciuria, renal calculi (kidney stones), or severe renal impairment where calcium levels are already elevated.
