Paracetamol, also known as acetaminophen, is a widely utilized over-the-counter analgesic and antipyretic medication indicated for the management of mild-to-moderate pain and the reduction of pyrexia (fever). Chemically classified as an aniline derivative, it is believed to exert its primary therapeutic effects through the inhibition of prostaglandin synthesis in the central nervous system, likely via the COX-3 isoenzyme variant, though its precise mechanism remains a subject of ongoing pharmacological study. Unlike non-steroidal anti-inflammatory drugs (NSAIDs) such as ibuprofen or aspirin, paracetamol possesses negligible anti-inflammatory activity and does not significantly inhibit peripheral cyclooxygenase enzymes, which results in a superior gastrointestinal safety profile and minimal impact on platelet aggregation. The drug is predominantly metabolized in the liver through glucuronidation and sulfation pathways; however, a small fraction is converted by cytochrome P450 enzymes into a highly reactive and toxic metabolite known as N-acetyl-p-benzoquinone imine (NAPQI). Under normal therapeutic dosing, NAPQI is rapidly detoxified by conjugation with glutathione. However, in cases of acute overdose or chronic excessive intake, glutathione stores become depleted, leading to hepatic necrosis and potential liver failure. Because of this narrow therapeutic index in high-dose scenarios, strict adherence to dosage guidelines—typically not exceeding 4,000 mg within a 24-hour period for healthy adults—is critical to maintaining its efficacy while mitigating the risk of hepatotoxicity.
