**Citicol (Citicoline) Summary**
**Indications**
* **Cerebrovascular Disease / Ischemic Stroke:** Accelerates recovery of consciousness and helps overcome motor deficits. Starting treatment within 24 hours of stroke onset increases the likelihood of complete recovery within 3 months.
* **Cerebral Insufficiency:** Relieves symptoms such as dizziness, memory loss, poor concentration, and disorientation due to head trauma or brain injury.
* **Cognitive Dysfunction:** Used in neurodegenerative conditions like Alzheimer's disease.
* **Parkinson's Disease:** Effective as an adjunct/co-therapy alongside standard treatments.
**Mechanism & Pharmacology**
* **Class:** Nootropic / Neuroprotective agent (essential intermediate in neuronal phospholipid synthesis).
* **Action:** Oral citicoline metabolizes into choline and cytidine, crosses the blood-brain barrier, and resynthesizes into citicoline in the CNS. It stimulates the biosynthesis of structural cell and mitochondrial membrane phospholipids, restores injured cell surfaces, and increases neurotransmitter levels (acetylcholine and dopamine). It also inhibits Phospholipase $A_2$ activation to reduce apoptotic and necrotic cell death.
**Dosage & Administration**
* **Oral:**
* *Acute Ischemic Stroke (Start within 24 hours):* 500–2000 mg daily.
* *Post-Stroke Cognitive Decline:* 1000–2000 mg daily.
* *Chronic Cerebrovascular Disease:* 1000 mg daily in divided doses, taken with or between meals.
* **Injection (IM or Slow IV [over 3–5 minutes]):**
* 1 to 2 injections daily, adjusted based on clinical severity.
* *Intracranial Hemorrhage:* If persistent, slow IV administration (30 drops/min) is required to avoid sudden increases in cerebral blood flow.
* **Geriatric Use:** No age-related dose reduction is necessary.
**Contraindications**
* Parasympathetic hypertonia (overactivity of the parasympathetic nervous system).
**Key Interactions**
* **Levodopa, Carbidopa, & Entacapone:** Enhances their therapeutic effects in Parkinson's disease.
* **Meclophenoxate:** Co-administration is contraindicated.
